Timely Follow-Up Matters

To support reduced readmissions, review the following reminders:

See patients within 7 days of SUD diagnosis/discharge
Assess for both mental health and substance use disorders
Encourage 2 or more follow-up visits within 34 days
Reinforce medication adherence when prescribed
Timely follow-up (within 30 days) helps reduce readmissions
